CAPS Paperwork

Free teletherapy is now available for students.

University of Nebraska–Lincoln has partnered with BetterMynd. To learn more and receive a referral to BetterMynd, please call CAPS at (402) 472-7450 and Press 2.

Please DO NOT complete any paperwork unless you have a scheduled appointment. Paperwork that is completed on a day without a scheduled appointment will be deleted at the end of the business day.

If you do have a scheduled appointment, please complete the appropriate paperwork at least 30 minutes BEFORE your appointment time on the day of your appointment; Initial Evaluation appointments require 40 minutes of paperwork time. Note that CAPS does not review your paperwork until right before your appointment.

CAPS forms are available online from 8 a.m.-5 p.m.

Help is available 24/7.

If you are experiencing a mental health crisis after hours:

  • Call Counseling and Psychological Services at 402.472.7450 and follow the prompts to be connected to an on-call therapist
  • If you are an imminent danger to yourself or others, call 911, call the University of Nebraska-Lincoln police department at 402.472.2222 or go to the nearest emergency room
    Nearest emergency room to campus is Bryan Medical Center West (2300 S. 16th St.)

If you have not been seen since August 1, 2022, you need to complete the forms in the YEARLY UPDATE PAPERWORK PACKET:

If you are scheduled for an INITIAL EVALUATION, you need to complete the forms in the "Initial Evaluation Paperwork Packet"

If you are scheduled for an INDIVIDUAL COUNSELING or CONSULTATION APPOINTMENT, you need to complete:

  • Informed Consent (if you have not been seen since August 1, 2022)
  • Remote/Telehealth Consent (if you have not been seen since August 1, 2022)
  • Standardized Data Set – SDS Form (if you have not been seen since August 1, 2022)
  • CCAPS 34

If you are scheduled for an ALCOHOL AND OTHER DRUG SERVICES APPOINTMENTS (Alcohol and Drug Psychoeducation Workshop, AOD Support Group, BASICS, CASICS, and Substance Use Evaluations), you need to complete:

  • Informed Consent (if you have not been seen since August 1, 2022)
  • Remote/Telehealth Consent (if you have not been seen since August 1, 2022)
  • Standardized Data Set – SDS Form (if you have not been seen since August 1, 2022)
